I will submit a hazard ID whenever the circumstances warrant it https://kcwaunch.com/hazard-id-and-near-miss-report/.

I will submit an incident report when any injury or damage occurs on a work site regarless of its severity https://kcwaunch.com/incident-report-form/.

I will submit a safety violation report if I see myself, or any other person on a site violating K.C. Waunch, Prime Contractor or Government safety regulations https://kcwaunch.com/safety-violation-report/.

I am aware that the K.C. Waunch Health-and-Safety policy, all health-and-safety forms, and the official Emergency Response Plan and Health-and-safety manual can be found at www.kcwaunch.com/health-and-safety.

I am aware that failure to follow these policies and procedures can be cause for termination of contract.
